DVRS IconQuality Insights Renal Network 3 is conducting a Population Health Focused Pilot Quality Improvement Activity (PHFPQ) in 2020 focused on encouraging gainful employment of ESRD patients (Increasing Vocational Rehabilitation Referrals and Utilization).  This project was rolled out in December 2019 for 25 New Jersey dialysis facilities.  The goal is to increase by 50% the referrals to an Employment Network or the New Jersey Division of Vocational Rehabilitation for eligible patients between the ages of 18-64 who currently are receiving Social Security Disability Insurance (SSDI) or Supplemental Security Income (SSI).  Additionally, there is a goal established to increase the number of patients enrolled in services by 1% of the denominator population.  
Participating facilities will be educating patients about the resources available to assist SSDI/SSI patients in their quest to return to gainful employment.  After the identification of eligible patients, facility social workers will be working with these patients to ensure they are aware of the services available and the process for becoming enrolled.
