If several staff members from one facility are attending the program together, then one staff member from the group should complete the sign-in sheet for either or both the 9:00 AM or 2:00 PM program.
- In order to obtain the contact hours for the program, please fax (609-490-0835) the Network the following completed forms:

- ANNA Program disclosure evaluation form and fax back to the Network at 609-490-0835
- Print the CPE certificate form (for your records only)
- Nurses and technicians
- Sign-in sheet for the program - 9:00 AM or 2:00 PM program
- ANNA program evaluation form and fax back to the Network at 609-490-0835
- Print the ANNA certificate form, complete and fax to the Network (and keep a copy for your records)
- Social workers
- Sign-in sheet for the program - 9:00 AM or 2:00 PM program
- CNSW Program evaluation form and fax back to the Network at 609-490-0835
- Print the CNSW certificate form, complete and fax to the Network (and keep a copy for your records)
