Quality Insights Renal Network Three stopped participating actively in the 5-Diamond Patient Safety Program on September 1, 2012, but continues to encourage facilities to utilize the 5-Diamond web site as a resource for dialysis staff education and to promote a culture of patient safety in the dialysis community.
